A new study shows that the hair loss treatment that’s helping men have their hair transplants may not only help them lose their hair, but also lead to an increased number of baldness or even baldness in a patient with a genetic predisposition to balding.
Researchers at Harvard Medical School found that men who took a drug known as finasteride had a higher risk of hair loss, compared with men who took an active drug known as minoxidil. In fact, there were no differences in hair density among the two groups. But researchers wondered how a combination of two drugs could lead to such a risk.
“What these results are telling us is that finasteride has the potential to treat a subset of men with a genetic disorder,” said Dr. Robert A. Schaffer, a professor of medicine at Harvard Medical School.

Dr. Schaffer and his co-authors found that men who took finasteride, like those taking minoxidil, had a higher chance of hair loss, compared with men who took an active drug known as minoxidil. In fact, the researchers also noted that a combination of finasteride and minoxidil reduced the risk of hair loss.
But some experts say finasteride, taken as an oral tablet, has been associated with more serious side effects, including a more than two-thirds increase in suicidal thoughts and suicide attempts. The risk of suicidal thoughts and suicide is greater in patients taking the drug.
Schaffer, of the University of California, San Francisco, said that finasteride, taken as a tablet, does not have the same side effects. However, he did not believe that finasteride was safe and effective in men who took the drug.
“Finasteride has not been shown to be safe or effective for use in patients with hereditary hair loss,” Schaffer said.
The drug can also cause other side effects, including a possible increased risk of certain cancers. And it can interact with other medications.
Other side effects that are more common in men taking the drug include a possible increased risk of diabetes, an increase in prostate enlargement and bleeding.
In a study of 8,000 patients who had undergone an operation for benign prostatic hyperplasia, researchers looked at more than 2,000 men taking finasteride. About 30 percent of those taking finasteride were not taking oral medications, and more than half had one or more of the following side effects:
Schaffer said that while these side effects can be managed by taking finasteride, he did not recommend it as a first-line treatment for men with hair loss. “I don’t think there is a need to try finasteride to treat men with a genetic disorder,” he said.
For now, the study’s authors, including Drs. Edward M. Schaffer and Robert A. Wilson, say the results are good news for men who are taking finasteride.
